@Sansemiano - Default all NPI options for vsync or frame limiter. Otherwise that vsync won't work.

@BenganJ - It might be worth giving note 3 a slight revamp?
Group gsync/freesync/adaptive sync together (3a) and put config.cfg vsync's at the bottom (3b).
And update these to better match the config.cfg line (i know i typed them originally and you pasted into your post, so blame me!).
uset r_vsync "0" = disabled
uset r_vsync "1" = full vertical sync
uset r_vsync "2" = 1/2 vertical sync
uset r_vsync "3" = 1/3 vertical sync
uset r_vsync "4" = 1/4 vertical sync

Ah, yes, defaulting of Frame Limiter and Vsync in NPI does the trick. The system now follows the r_vsync setting in config.cfg. I tried 0, 2, 3 and 4 resulting in >100 fps, 60 fps, 40 fps and 30 fps.
At 30 fps the panning of the camera gives horrible stutter in the image. At 40 and 60 that is definitely less. At the highest unrestricted fps it's best. It's just gradually improving with a higer framerate.
I does however make a HUGE difference on the load of the GTX 1070 giving more room for higher graphics settings.

@Sansemiano - You have max prerender frames at 1 - set to default 3 to see if that helps the panning issue.

Just tried with prerendered frames at 3. Makes no difference. Still a bibbery stuttery panning at 30 fps. Even the mouse works less precise with this setting.

There's no need to strive for 30 FPS, the thing that matter is that you get a steady FPS at the set limit.

True. We do not all have to live in the same frame rate.
But, I do think, that the higher the frame rate the better.

That's damn correct, AS LONG as it is stable!

at the highest possible graphics quality, which means that in most
cases you will have to compromise.

I'm new to this forum but Im having a issue with the settings for multimon and the nvidia inspector wen in game my graphics scaling is 100% my fps is about 20 and below but if I change in game scaling to 125% my fps is in the high 50 to 60 fps but picture is not very good am I doing something wrong
