As of today's first post, the monitor and game were set to 2560x1440.

Has this been tried/checked?
All NPI vertical sync settings need to be defaulted.
I see in your NPI pic - Vertical sync tear control - adaptive - default this.

uset r_fullscreen "1"
uset r_mode_refresh "0" <---- try setting 60
uset r_mode_height "1440"
uset r_mode_width "2560"

uset r_vsync "2" <--- OK

That's a rest from the "poor mans adaptive sync", but does not mess up my system!

EDIT: uset r_mode_refresh "0" does no harm on my system either, set to default!

uset r_vsync "0" <--- to force off. Now do you get 100+ fps in game?
If not, then something else is controlling vsync and not the game.

Depending on monitor type or make perhaps the game is reading the hz incorrectly?
hence set to 60 to test.

When it is set to 0, it is 122 or more. When driving in a game, it will drop to 80 cars.

Without using fps limiter in NPI,
virtual sync = 1/2 refresh rate
(30fps)
Is not a substitute?

Can you try uset r_vsync "3" too and see what FPS you get?

No, then uset r_vsync "2" would have given 45fps! This is getting weirder by the minute!
Please, try the last one too uset r_vsync "4", what do you get then?
