@Draftlight
Glad it worked out for you! I'm not so well versed in the NPI or game graphics settings, so maybe someone else can give you recommendations. Although I would say a constant 60 FPS is going to be pretty tough, it might be more realistic to aim for a consistent 50 or 40 FPS instead.

If your gpu usage is still 99% when it dips then it can't render the amount of frames you want at that point.
If gpu usage drops to say 70% at the dip then your cpu is now the bottle neck and can't feed the gpu quick enough because it's busy doing other stuff, AI traffic etc or the frame detail that the cpu has to generate at the dip point has increased and therefore takes longer before it can pass to gpu to render.
It's better to lock your frames to a lower limit to allow the gpu to increase it's usage when required and therefore avoid dips.
Drop mirror distance & shadow quality to minimum and re-check.
If that's better then increase them to find the sweet spot.

Yeah looks like my GPU is still bottlenecking, even with the Mirror distance on 100 and shadows on low.
It even started dipping down in London to 20 FPS again.

Ill finish the delivery job and then see what happens if i remove the mods again since i have a base now that i can properly work on.
Edit: Even without any mod enabled, still the same as above, weird, im starting to think that i do need to upgrade my GPU which i wanted to avoid

@Draftlight - If you want to, zip up your current game log and attach and i'll have a look at exactly what you have set in game.

Re: [DX11] Help regarding Nvidia Profile Inspector (NPI)
@IamIrie - If you mean like this --> viewtopic.php?f=23&t=1865
Try config.cfg --> uset t_averaging_window_length "60"
As you say 30/45/60fps that looks like vsync can't maintain 60fps and drops to 30. Don't know why you would have 45 in there.
This AA is demanding, if your setup could run default game with 400% scaling then it should run this AA at 4x and 100% scaling ok.
If not, try 2xAA.

Re: [DX11] Help regarding Nvidia Profile Inspector (NPI)
How do I not have all NPI parameters???
Re: [DX11] Help regarding Nvidia Profile Inspector (NPI)
@martinhf02 - You must have deleted the profile in NPI or in NCP.
Just re-install the driver (use clean install option not express) should get all available profiles back when using NPI.
Or create a new profile in NPI, 'ETS2' and add Eurotrucks.exe to it and then add the step 1 settings.
@andrey2ko - Read the top of the first post, some options aren't available in all NPI versions, it's just the way it is. Use NCP to change the options you can't in NPI.
