@toni1958 - Overclocking isn't worth it, extra heat and strain for little gain. 10% increase is easy!!!
But what is 10%... at 24 fps it's an extra potential 2 fps.
You need to lower game settings to increase fps or as a test to run 4xAA, if you want to, try this:
Nvidia surround - pick monitor resolutions 1280x720 instead of 1920x1080 - its the same aspect ratio.
In game resolution will be 3840x720.
Antialiasing - 4x Multisampling
Antialiasing - Transparency Supersampling - 4x Sparse Grid Supersampling
Then remove the blur it'll create by:
Increasing NPI Texture filtering - LOD Bias (DX) to '-2.0000' or more. -3.0 may be too sparkley.
And NCP Sharpening Value to '0.80' or more?
Only you can decide if this it suitable option for your eyes.

Taken a while to come back here as I was at lunch when I posted earlier (WFH benefits :)), and just spent the last 1.5 hours trying literally everything.
I'm not sure how/if that line was truncated, lost track a bit now having tried so many things, but I'm fairly sure I just had "-homedir "G:\" in the line which is how it has been for a long time. My profile and mods load, and both my config and multmon_config were in G:\Euro Truck Simulator 2.
I tried removing homedir from the launch options;
Symbolic link %USERPROFILE%\Documents\Euro Truck Simulator 2 to G:\Euro Truck Simulator 2
Deleting the symbolic link folder and starting up ETS2 so it sets set up a vanilla profile there - the triple monitor config does give me three images
Either of the above should effectively make all exactly as my ATS install is, so this isn't making much sense by this point.
If I revert to G:\Euro Truck Simulator 2 again, and put the triple multimon_config in there - again I do get three images. It may be me but it looks different when I toggle the NPI config so it is doing something, but not AA.

Okay, interesting! BUT please, be careful with those "-signs in the -homedir argument.
As you are trying to achieve G:\ you don't need any "-sign at all. They are needed only
if you have space(s) in that path! On the other hand, I saw that the game obviously got
it right in the end. A -homedir "G:\" should be OK though! Let
us check your settings in NPI and config.cfg once more. I'll be back!

I am truly amazed, was trying different configurations after your last comment about lowering some of the ingame settings, I only had the mirrors on medium for all 3 settings and dropped the quality and resolution down to low and.....its a completely different game and GPU drops to 80 at times, and to be honest, I can't see any difference in the mirror quality etc. I will now try your other suggestions and who knows.... 4xAA here we come haha
Yes I did see the note about saving the profile but never gave it a second look as I thought the profile would always be in inspector, I didn't expect it to get wiped when I uninstalled the GPU driver, silly me, have now saved somewhere safe as I'm not gonna get caught a second time :-)

No, NPI alters the Nvidia driver settings directly and doesn't save those settings anywhere, unless you yourself EXPORT them.
So, a simple driver update will erase those settings and for that reason you MUST do that EXPORT, otherwise you will have to
manually configure every time after an upgrade. BTW, setting Mirror Quality to anything but Medium will remove AA in the
mirrors, but if that's OK for you .....! That's also the reason why you get a better FPS, as the AA calculations will not take place!

When I configure surround in NCP I only get the choice of 5840x1080 until I click on enable. I can then choose different settings but whatever I choose apart from the recommended 5840x1080 means that I get nothing in either of the side screens and everything is grossly oversize on middle screen that I can hardly get to any of the buttons. I also have to choose 5840x1080 ingame because even 1 setting lower and everything is stretched superwide but still over 3 screens.
Any ideas??

I don't know whether its coz I am only using 2xAA but I see very little difference if any of objects in the mirrors with them set on low resolution and quality, I parked near a truck so I could see it in mirrors and tried both settings. I'm going to try 4xAA to see if it can handle it :-)

Could be, as 2xAA has less impact! Don't forget to limit at FPS = 30, at least when trying 4xAA!

Ha ha, limit to 30fps, you must be joking, with every ingame setting on LOW my GPU is running at 100% all of the time again and FPS is down to 20-22 out in the country, so in town, well I guess I would stop :-)
Maybe JHTrucker can think of another way to get my screen resolution down which then might help everything else.
