@Pancreas - Did you download my Triple.AAv4 file? step 2 - updated March 29th.
If that doesn't help.
I would reset NPI settings for game and just do step 1 settings and rename config.cfg so game generates a new one then add step 3 settings only.
Remember this is about high quality image rather than increase FPS.
But you can reduce your triple resolution - in game pick next lowest and see what it looks like. Sharpen to de-blur it.
Or use 2x [2x Multisampling]' '2x Sparse Grid Supersampling' like i have to.
If you're running promods etc then.... your stuck.
How would you like a triple setup that only uses half of the left monitor, full center & half right monitor? So that's 2/3 of full screen res and will gain one screens worth of FPS back.
I'll config one if you like the idea of black areas far left and far right. What FOV do you use?
EDIT: I remember now... you had 1440x900x3? and now changed to 1920x1080x3. Quite a step up for triple AA.

@BenganJ - Well wild guesses gave way to serious thinking the past couple of days... DSR is a good scaling solution for single monitor guy's so i couldn't let multi's miss out.
So i have multi's working with horizontal scaling, can't increase height, but can't decide if the FPS hit is worth the subtle image enhancement. Should be better for tall thin things like distant lamp posts etc but i can't test in motion... too low FPS, 14 at 200%!
Given what the date is tomorrow or today for you, i'll wait a day to publish in case high end guys need to really push their cards.

But, one thing to note in your note, Just add after:
So, anyone using a 120hz monitor and can run above 40 FPS can use vsync=3 for 40 FPS tear free.
Don't use NPI frame limiter or vertical sync options, just default them. <---- this line ***

@BenganJ - Yes, the important bit is all others to be default / "app selected" so just in game vsync is in control.
