This is so DAMN odd! No Frame Rate Limit in action at all. So, then I suggest that you
take one step back and set the following again.
1. In NPI set Vertical Sync to Use 3D application setting.
2. In config.cfg set r_vsync "2".
Then please, another short video!
EDIT: Did you REALLY restarted the game after having set the Vertical Sync in NPI?

I'm ABSOLUTLY sure I restarted the game. I even restarted my system. I'll do the steps again.

You should NEVER need to restart the computer, JUST THE GAME, and the best is
to QUIT the game, do the settings and THEN start the game again!
EDIT: Did the setting in game for Refresh rate say Default refresh?

sorry if i interfere, but are you using a laptop or a desktop?
I get the same kind of tearing on my laptop
Proc: i7-9750H 2,6 GHz (max. 4,5 GHz)

This has nothing to do with tearing, he can't get the Frame Limiting to work through NPI or NCP,
only through the game itself and that is DAMN odd! Tearing you will get if you as his video showed
have a heavily varying FPS!
