Nice that you have found your way back to the keyboard, hope it didn't hurt too much when you fell off your chair glad you've had a serious laughing session at my expense :-) If you only knew how long it was taking me to make a change in NPI, save to game exe, save to extra folder for safe keeping, verify the game files and then re aquire through steam.... so much easier doing it the correct way, took me 2 days to reinstall all my racing games too.
Now I have done everything correctly the game is working superbly, so smooth(except for the annoying stutter every few mins) but I can live with that. There is no jerkyness, apart from the occasional lag in towns with lots of traffic, so on the whole, I am well pleased.
I have found that changing the "max pre rendered frames" to 3 does make it less smooth so have set it back to "use the 3D app setting", everything else in NPI, NCP and config.cfg is as per your suggestions.
I have a question which you may be able to help with.... graphics card is running at 50-60% most of the time and rises a bit with traffic etc, FPS is 29-30 most of time but drops with traffic, buildings etc to 22-23. If my card is only using half to three quarters of its potential, why do I get such massive FPS drops?
I tried changing the "vertical sync tear control" from "standard" to "adaptive" but this makes the FPS go up to 50+ and then still drops to 20 something with obviously a bigger impact on the game graphics.
If it is ok, I will attach my multimon file and perhaps you could have a look at it when you have the time.
Going back to the head out of the window and "trackir", you referred to using the same files or something as in the game. When I had it working (of sort) before I set it up in "facetracknoir", using nothing from the actual game itself, if that makes sense?

Are you by any chance using r_vsync "2" now in config.cfg? I suspect that's why
the tear control in NPI doesn't work correctly. I use half refresh in NPI instead
and then adaptive doesn't make the sync to disappear! Also, have you set the
config.cfg t_averaging_window_length to "60"? It will smooth out some of the
stuttering you might have now!

Good evening, yes I am using r_vsync "2" in config.cfg, I thought I would try it as it didn't seem to work properly last time so perhaps it is still faulty. I'll try going back to NPI settings now and see what happens. I can also confirm that averaging window length is set to 60.

Don't take as if it's the law, test it and see if it works! You can
always revert back to what you have now!

If my GPU was running at nearly full capacity all the time like the original one was I can see why I would get the drop in FPS as it got to 100% and still had more work to do, but as this one seems to have plenty of scope in reserve I fail to understand what the problem could be

I noticed earlier you said you had only just started using version 1.39, have you noticed any considerable difference with regards to your settings?
I have noticed that when I set vsync to 2(config.cfg) my ingame vert sync setting is "Economic(saves energy for notebooks), as far as I can remember 1.38 was only enabled or disabled. I have tried both of these settings but smoothness is better with economic.

Nope, your memory is worse than mine, mate! r_vsync "2" has always been Economic!
