I have confirmed how to set up your monitor AD27QD.
Turn on the free sync on your monitor.
My OS is Japanese and NCP is also Japanese, so you may not understand it even if you look at it, and the setting items are also different.
Also, the version of NPI is different.
I will post all the settings in a video later.
Also note that I'm using 60fps.
You should also be able to run ETS2 at 60fps and above.
If you want to use the NPI settings and avoid the FPS drop you want, my tests only allowed it to be below 1/4 refresh rate (36fps or 30fps) at 144Hz. (For 2070Super)
Last but not least, fps is lost on almost every map, including DLC. I think the only way to avoid it is to fix it below at least fps. Don't worry about losing 10-15fps from time to time.

FPS is not an issue.. the issue is lag in game and the game is not at all smooth...if you can tell me what are the below mentioned settings at present.I can implement it till the video comes.

Edit : Here are my setting

NPI

Frame rate limiter V3 : Off
Vertical Sync : 1/3
Vertical sync tear control : Adaptive

NCP

If any change you made : No Change in my case

config

uset r_mode_refresh "0"
uset r_vsync : 0
uset t_averaging_window_length : 96

In game

Vertical sync : Disabled
Refresh rate : Default refresh

It contains the G-Sync settings that I looked up on domestic and international sites. I don't know if it's right or wrong, but I'm doing well.

NPI
maximum pre rendered frames = 1
ultra low latency cpl state = Ultra
Please note that is included.

Processing to HD version.
